19 Direct Service Kenmore Hall Case Manager Job Description:Some of the responsibilities are: 1. Assist the Program Manager on a daily basis in the operation of the social services department. 2. Participate in initial interviews and orientation with prospective clients referred by DHS assess the substance abuse needs of potential clients. 3. Establish and maintain individual case records on assigned clients, including accurate and up-to-date documentation of contact with clients, treatment plans, room visits, medical reports, and entitlements. 4. Ensure that case records are maintained in a neat and orderly fashion. All records must conform to the guidelines established by the Kenmore/HSI and DHS. 5. Assist the program Manager in monitoring and maintaining all tenant and administrative files on a daily basis. 6. Implement all policies and procedures in the Resident/Tenant handbook. Enforce the Kenmore's "House Rules" with clients and to work to resolved related incidents. 7. Provide counseling assistance, Information and/or referrals to clients regarding, but not limited to, the following issues: legal, guardianship, medical, financial, education, job search, support groups, recreation, food programs/pantries, clothing, and donations. 8. Abide by and uphold all federal, state, City and county regulations regarding the confidentiality of client information. 9. Address independent living skills with clients as needed. 10. Monitor all client's needs and report accordingly to the program manager. Inform the program manager of all tenant issues that require immediate management attention. 11. Provide crisis intervention, conflict resolution, and mediation as needed. 12. Submit weekly reports to program manager in a timely manner. 13. Assist with tenant meetings as needed. 14. Other duties as assigned by the program manager. Specific Education, Certification and Skill Requirements (if applicable): 1. Bachelor's degree with Human Services experience ORMaster's degree in Social Work 2. Bilingual a must. (English and Spanish) 3. Computer literacy 4.
